Passed away peacefully surrounded by the love of her family at Southlake Regional Health Centre on Friday, October 13, 2017 at the age of 26 years. Dearly loved mother of Nichole and Kaleb. Cherished daughter of Gabriele Eisenmann and Mike Jay. Loving sister of Michael Jay (Stephanie).
